S.C.A.N.

The Service Coordinator and Navigation (“SCAN”) Program supports youth at the highest risk of exposure to violence, violent victimization, and active involvement in violence who live in the Grand Crossing and Chatham communities, serving residents of the following zip code areas: 60619, 60620, 60621 and 60637.

Program Outcome Goals include:

    • Increasing youth self-efficacy. Ensuring youth have a network of supportive, caring adults.
    • Ensuring youth have a network of supportive, caring adults.
    • Supporting youth in meeting their needs and goals to build stability in their lives.
    • Re-engaging and maintaining engagement in school or work.
    • Obtaining safe and stable housing.
    • Reducing justice involvement.
    • Reducing incidents of violent victimization.

Success across these outcome goals will contribute to an overall reduction in youth violence involvement and violence at the community level, thereby helping the City of Chicago build a wider evidence base on its impact to reduce occurrences of community violence.

The S.C.A.N. Program model aims to reduce youth involvement in violence and improve well-being by providing youth with services, stability, and a network of community support to guide them towards successful adulthood.

The framework behind the name of our organization came from a sermon titled “A Knock At Midnight” preached by the Reverend Dr. Martin Luther King Jr
